Basic Guidelines for Filing Proof of Claims

• Use only Bankruptcy Official Forms.
• Only file claims in open cases.
• Do not file claims in no-asset cases. If the trustee determines there are assets, you will be notified of the deadline for filing a Proof of Claim.
• Ensure the necessary documentation is attached.
• Ensure personal identifiers are redacted to ensure compliance with Fed. R. Bankr. P. 9037. This requirement applies to all documents, including attachments.
• The creditor's Name and Address cannot exceed five lines per creditor.
